Give a Healthcare Proxy Access to Your MyChart Account

Whether it’s a caregiver, a family member, or someone you trust—MyChart lets you share access securely.

Log In to MyChart
Go to mychart.chsbuffalo.org and sign in
Navigate to ‘Sharing’
In the main menu, select “Share My Record” or “Proxy Access”
Choose ‘Friends and Family Access’
Select the option to add a new person
Enter Proxy Information
Fill out their name, email, and relationship to you
Set Permissions
Decide what they can view or do (medical history, appointments, messaging, etc.)
Send Invitation
MyChart will email the proxy with instructions to accept access

In the app store (Google Play or the Apple App Store) search for the CH MyChart app.
Download the app to your device
After the download is complete, open the app, and log in using your Catholic MyChart username and password. If you do not have a username and password, select Sign up now to set up an account.
