Whether you're ill or living with a physical challenge, it can be difficult to leave your house to get the care you need. That's where we come in – Catholic Health's home care services make life a little easier by bringing our trademark compassionate care to you.

Receiving home care means that a nurse or aid will come to your home to provide healthcare, personal assistance, or other needed services. You may benefit from home care if:

  • You have been discharged from a hospital or other facility, but require additional assistance at home
  • You have a medical condition or physical challenge
  • You need assistance with personal care, household chores, or other daily living activities

Home Care Services 

Catholic Health's home care services are provided by McAuley Seton Home Care and Mercy Home Care, and are available to residents of Erie County and Niagara County.

Skilled Nursing

Skilled nursing is the highest level of care a patient can receive outside of a hospital. Registered nurses, licensed practical nurses, and other healthcare professionals provide 24/7 care for seniors who are recovering from an illness, injury, or living with a chronic condition. 

Home Rehab Therapy

Specialized rehab services offered in the comfort of the patient's home include physical therapy, wound therapy, speech therapy, and services of licensed social workers and registered dietitians. 

Assistance with Daily Living Activities

The nursing aides in our home care programs provide assistance with daily living activities, or those essential skills that satisfy our most basic needs. From housekeeping tasks to personal grooming habits and daily companionship, we're there to support you when you need it most in your life. 

LIFE (A PACE Program)

LIFE is an adult care program for participants that still live at home, but may also be in need of more supervised care. Our services promote a healthy, engaging lifestyle for adults – the right kind of care, made available at just the right time. 

Cardiopulmonary

Our home care services include specialized post-op care from cardiac surgery, as well as disease management for conditions like congestive heart failure, COPD, pneumonia, and other cardiopulmonary diseases. 

Infusion Therapy

Catholic Health's own infusion pharmacy provides compassionate care for patients receiving IV therapy services. 

Nurse-Family Partnership

Nurse-Family Partnership creates a unique support system for first-time mothers, with the belief that our services create a better future for both mom and baby. Our program fosters trust between participating mothers and our trained nurses, who provide assistance throughout pregnancy, and up until the child's second birthday. 

Starting Off Right Program

The Starting off Right program is here to provide support to mother and baby at no additional cost. This is a Catholic Health home based program that works in conjunction with Independent Health. We are here to support by providing assistance from pregnancy through the first 6 months after the birth of the child.

Mother & Baby Home Care

Our home care nurses are passionate about providing comprehensive home care to both mom and baby, especially in cases of high-risk postpartum or if your baby requires extra care. 

Palliative Care

Palliative care provides medical care and relief from the pain and stress of a chronic or terminal illness while receiving qualified skilled services. Palliative care at home can reduce hospital visits by bringing your nurses and doctors to you when you are ill but it's not an emergency.

Spiritual Care 

Spiritual care is offered by our team of professional chaplains and pastoral associates - part of your care team. These caregivers provide spiritual, emotional, and psychological support to patients and families of all denominations.
