Catholic Health home care services make life a little easier by bringing our trademark compassionate care to you. 

When you receive home care, a nurse or aide will visit you at home to provide medical care or other services. Home Care is generally recommended if:

  • you have been discharged from a hospital or other facility but require additional assistance at home
  • you have medical conditions or physical challenges
  • you need assistance with personal care, household chores or other activities of daily living

 

Services

Skilled Nursing  – Skilled nursing is the highest level of care a patient can receive outside of a hospital. Registered nurses, licensed practical nurses, and other healthcare professionals provide 24/7 care for patients who are recovering from an injury, illness, or living with a chronic condition. Home Rehab Therapy – Specialized rehab services at home include physical therapy, occupational therapy, speech therapy, and treatment from registered dietitians, licensed clinical social workers, and certified wound therapists. Learn More > 

Assistance with Daily Living Activities – Daily living activities are those essential skills that satisfy our most basic needs. The nursing aids in our home care program provide support where you need it most in life, from housekeeping tasks to personal grooming habits and daily companionship. Learn More > 

LIFE (A PACE Program) – LIFE is an adult care program for participants that still live at home, but may also be in need of more supervised care. Our services promote a healthy, engaging lifestyle for adults – the right kind of care, made available at just the right time. Learn More >

Paying for Home Care

Our services may be covered by Medicare, Medicaid, HMOs, private health insurance, or on a private-pay basis.

Accreditation is regarded as one of the key benchmarks for measuring the quality of an organization. Accreditation is a process of review that demonstrates our ability to meet predetermined criteria and standards of accreditation established by a professional accrediting agency.

On-site surveys are conducted every three years by industry experts. A comprehensive review is conducted of organizational structure, policies & procedures, compliance with federal/state/local laws, leadership, patients’ rights & responsibilities, fiscal operations, human resource management, provision of care, patient records, quality outcomes, performance improvement, infection control and patient/employee safety.
