  • Dean’s Letter
  • Medical School Transcripts
  • COMLEX/USMLE Part 1 & Part 2 scores, including PE/CS
  • 3 Letters of Reference
  • Physicians who graduated from a LCME or ECFMG accredited college who wish to apply to our Osteopathically Recognized Family Medicine program are required to:
    1. Provide a personal statement explaining their interest in our recognized osteopathic family medicine program.
    2. Complete a personal interview with osteopathic interview committee.
    3. Successfully complete a basic osteopathic Principles and Practice training course prior to residency start date. For example, “Osteopathic Principles and Practice for the MD” through MSUCOM CME or “Osteopathic Medicine for All: CME/GME” through Ohio University Heritage College of Osteopathic Medicine, or another comparable course giving a brief intense update to allow MD applicants to gain a better foundation to the history of the profession, nomenclature, and basic principles for both diagnosis and treatment.
